To request your certificate, please read the information below carefully and fill in the form. You will also be required to provide the following documents:

Copy of Level 1 certificate
Copy of 5 letters of completion (one for each module, provided by your Lead Trainer)
LEVEL TWO Practitioner Certification Form

Please choose between a Digital L2 certificate or a Printed L2 Certificate. For the hard copy printed version shipped directly to you, there is a fee of US $35.00.
